Eligible Milford Schools students in kindergarten through third grade can receive free eye exams and prescription glasses, if needed, through school visits beginning Monday, Nov. 2.

The district announced a partnership with HealthSource of Ohio to bring its Mobile Vision team to Milford school buildings from Nov. 2 through Nov. 10. The free services for younger students come through HealthSource's partnership with the Ohio Department of Health. Students in other grades can also receive exams, and most insurance plans are accepted.

Families must complete a two-step registration before their child can be seen. The first step is an online request form with the child's full name and date of birth. Families will then receive an email by the next business day with a link to complete consent and health history forms. Both steps must be finished for a student to receive services.

The initial enrollment form is available on the Milford Schools website. Families with questions can email HealthSource of Ohio at [email protected].
